We are conducting a survey on awareness of pesticides in Tanzania.

Pesticides are chemicals that are applied on crops and plants to control pests. Pests are insects or animals that destroy plants, food etc.
This questionnaire seeks to understand how aware people, in Tanzania, are of pesticides and their related health effects.
This survey will take about 15-20 minutes of your precious time to complete. We greatly value and appreciate your time.
All data is collected on the basis of anonymity i.e. no personal data such as name, and date of birth shall be collected.
This survey can be filled in by anyone living in Tanzania (working, studying, etc.).
This survey is accepting responses from 3rd March 2014 to 30th April 2014.
