HIV/AIDS: New blueprint charts PEPFAR's future

JOHANNESBURG, 30 November 2012 (PlusNews) - In its newly released "blueprint," the US President's Emergency Plan for AIDS Relief (PEPFAR) charts the next phase of the country's response to the HIV epidemic. In it, the largest funder of HIV efforts globally will focus on HIV prevention, new technologies, women and girls, and the most at-risk populations (MARPs).

This year will be US Secretary of State Hilary Clinton's final year as head of the US State Department, under which PEPFAR falls. At the International AIDS Conference in July, Clinton asked PEPFAR head Eric Goosby to create an action plan for future administrations, policymakers and implementers.

On 29 November, Goosby and his team presented the result: PEPFAR's new five-point blueprint. The need for sound and scientific investment is an overarching theme in the 53-page document, which outlines four major aims: saving more lives, smart investments, sharing financial and programmatic responsibility, and driving results with science.

Under this plan, the agency will support medical male circumcision; targeted services aimed at MARPs such as injecting drug users and sex workers; and research into technologies such as microbicides, vaccines and point-of-care tests. In the blueprint, the US government commits to using diplomatic channels to help create enabling environments to allow MARPs to access health services. It also prioritizes using health diplomacy to promote the health and human rights of same-sex populations and women and to advance gender equality.

The need to fund new scientific innovations in the fight against HIV, and to address gender inequality, was also echoed at the launch by former South African health minister and now chairperson of the African Union Nkososana Dlamini-Zuma. After being elected chairperson in July, Dlamini-Zuma said that health would be high on her agenda.

"It is encouraging that some of our countries (within the African Union) are already putting a certain percentage of their budgets aside for innovation, not only around issues of HIV," she said. "We will have to work hand-in-hand with partners to empower women because we can't get to that point of an HIV-free generation without empowering women to be able to take and make the right decisions."

AIDS-free generation

The blueprint is part of the US push towards an "AIDS-free generation," which Clinton has defined as one in which almost no children are born with the virus and in which children grow up at a reduced risk of contracting HIV later in life. Speaking at the Washington, DC, release of the blueprint, Clinton stressed that an AIDS-free generation is now within reach.

"As we continue to drive down the number of new infections and drive up the number of people on treatment, we will eventually treat more people than become infected," she said. "That will be the tipping point when we get ahead of the epidemic and an AIDS-free generation will be in sight."

HIV activist Florence Ngobeni-Allen spoke of PEPFAR's role bringing HIV treatment into South Africa. Ngobeni-Allen, who is HIV-positive, lost her first daughter and husband to AIDS-related illnesses in the mid-1990s; at the time, HIV treatment was not yet available in South Africa's public health system.

"I started working as a counsellor at Chris Hani Baragwanath hospital during the time when there were no drugs," she said. "The worst part was hearing those screaming babies crying and looking at them and thinking, 'they are following my daughter.'

"I couldn't give them anything but my hope and my smile," she added.

Now remarried and mother to two HIV-negative sons, Ngobeni-Allen says her children are part of what she hopes will be an AIDS-free generation.

Fixing the spend

The blueprint commits PEPFAR to strengthening its supply chains and business processes and to integrating programming with other donors and development partners. This may be a response to long-running criticisms from activists and global health professionals over inefficiencies in distribution and within recipients.

In April 2012, HIV activists in Kenya took to the streets after about US$500 million in unspent PEPFAR funds were discovered that had reportedly been "stuck in the pipeline."

In the blueprint, the US also commits to funding the Global Fund to Fight AIDS, Tuberculosis and Malaria as it approaches its next replenishment meeting.

This World AIDS Day there are hopeful signs to report. Last week the
Joint United Nations Programme on HIV/AIDS (UNAIDS) announced that
there has been a 13% reduction in TB/HIV deaths in the last two years.

This is encouraging, but we are far from where we should be. TB
remains the leading cause of death among people living with HIV. At
the current rate of progress, and without tremendously accelerated
efforts, the goal of reducing deaths from TB among people living with
HIV 50% by 2015 - agreed to by UNAIDS and the Stop TB Partnership -
will not be achieved.

We know what should be done. Ideally, people should be offered "one
stop service" for HIV and TB care. Every country can move in that
direction through a tailored approach. TB programmes need to provide
HIV testing; and HIV programmes should do TB screening and needed
follow-up. And there should be active outreach into the population to
offer both HIV testing and TB screening.

I congratulate the many countries that are providing HIV testing in
the context of their TB programmes. In 2011, more than two-thirds of
TB patients in the African Region had a documented HIV test result.

However, I must challenge many of those same countries to urgently
scale up their efforts to make TB services (screening, prevention,
diagnosis, treatment) available through their HIV programmes. Today
not nearly enough people living with HIV are receiving these services.

I appeal to the entire HIV community to make this issue your own. You
have called for zero new HIV infections, zero discrimination and zero
AIDS-related deaths. You have also called for an AIDS-free generation
via the new global AIDS blueprint. These aspirations will never be
achieved without confronting TB and reaching with equal zeal for zero
TB deaths and zero new TB infections.

I call on all people in the TB and HIV communities to integrate their
activism - just as we must integrate TB/HIV care.

UGANDA: Profiles of infidelity, HIV vulnerability

KAMPALA, 30 November 2012 (PlusNews) - Married or cohabiting couples are at a higher risk of HIV infection in Uganda than their single counterparts, with some studies finding that as many as 65 percent of new infections occur in long-term relationships. The prevailing culture, a hybrid of traditional mores and more modern, western values, accepts - even expects - men, and increasingly women, to have a "side dish" - a euphemism for a sexual affair.

IRIN/PlusNews spoke to a few women in the capital, Kampala, about how they protect themselves from HIV in non-monogamous relationships. All names have been changed at the women's request:

Grace, a married musician

"He does cheat a lot. He is never at home. He comes back home late every day. His phone is full of messages from women.

"I can't do much. We have never gone for an HIV test together, and I can't ask him for us to start using condoms, yet we are married. If I try to deny him sex, that will be trouble and a disaster at home; we would not talk for about two weeks or a month. He would suspect that I am cheating on him.

"I do cheat. However, I always use protection. I am not sure whether he [husband] protects himself when he goes out with other women.

"The 'side dish' [public HIV] campaign messages really touch my heart. That is why I use condoms if I am cheating on my husband."

Janat, a banker, wife and mother

"He has cheated before, and I believe he is still cheating. Every man gets tired of one woman and wants new stuff. Even me, I get tired of him and need a new person for a change. I do monthly HIV tests alone. He doesn't want to do the HIV tests.

"We normally use condoms, but not for HIV protection; we use them when I am about to start my menstrual periods and when I have fungal infection like candida.

"It's difficult to deny a man sex. If you do so, he will leave you at home and go to look for other women to satisfy his sexual desires.

"Of course, I have cheated and still continue to do it. It's impossible and unfair for an individual to have one sexual partner for the entire life, even if you are married. It's normal to cheat while in marriage. Variety is the spice of life.

"I use condoms with the side dish... to protect myself from contracting HIV. I also move with my HIV test kits [bought from a local pharmacy for about US$1.10]. If I need to have live [unprotected] sex with the side dish, we first test before we do it live.

''Of course I have cheated and still continue to do it. It's impossible and unfair for an individual to have one sexual partner for the entire life, even if you are married. It's normal to cheat while in marriage. Variety is the spice of life''
"The [marital faithfulness] messages are good, but cheating among married couples is not about to go anywhere. The sexual network will continue, but people need to be careful and mindful. HIV is real. They should always protect themselves."

Damalie, an insurance official, wife and mother

"He has cheated on several occasions, and he is not about to stop. I stopped having sex with him. I couldn't continue to risk my life. I told him we have to put on hold [sex] until further notice when I feel I have rebuilt my trust in him. That angered him so much. He walked out and formalized [marriage] with another woman.

"I have never cheated on the father of my children. As for testing for HIV, I am not concerned since he walked out.

"It was unusual for married women to cheat in the past, but it's so rampant these days. Women have been emancipated, independent and empowered. So they feel that what men do they too can do. But...as they cheat, they should remember protection. "

Deborah Nabirye, a parking attendant, co-wife and mother

"Despite us being two wives, the man still cheats on us. You can't imagine - I caught him with the housemaid. We seem old to him now. He wants young girls... [he] is cheating because he has a lot of money.

"I can't restrict or refuse him from going out. I do the HIV test alone. I have tried to plead with him for us to go for an HIV test, but he has always refused. Whenever I can, I get free HIV testing in order to know my status in order to plan for my children.

"As a married woman, I can't start asking my husband for us to use condoms. I don't have trust in condoms; I have a feeling the virus can go through them.

"I don't cheat on him. I really love him despite his cheating on me. I have several men who are chasing after me, but I can't cheat. I fear I may be the one to bring the virus. I don't want my children to suffer as we did. I lost my parents to HIV, and we suffered. We didn't have anyone to pay school fees and provide the basic needs. If I didn't go to [a tertiary-level] institution through government sponsorship, I would have struggled to raise the [money for] tuition.

"I pray to God daily, not only to for him [my husband] not to get HIV but to have mercy for him to change his sexual behaviour. It's only God who protects. I can't do anything."

The Banker Magazine, a publication of Financial Times, has named United Bank for Africa Plc (UBA), the 'Bank of the Year', Africa at an event held in London.
The bank also won the country awards for Senegal and Cameroon, bringing the total number of awards received by UBA to three.

A statement from UBA yesterday, said the financial institution carted home the regional award for the continent, joining the ranks of other regional winners like Scotiabank-Global Americas, Nordea-Western Europe, Raiffeisen Bank International -Central and Eastern Europe, DBS-Asia\Pacific and Qatar National Bank-Middle East.

"The 'Bank of the Year' is selected based on the overall performance of the financial institution and the opinion of leading financial analysts from the world's financial markets. Winners were recognised and presented with awards in the presence of over 450 leading bankers across the globe," the statement explained.
The awards, according to UBAM further attested to its growing recognition "as the leading pan-African financial services institution and as Africa's global bank."

The awards were received by the Bank's Deputy Managing Director, Mr. Kennedy Uzoka; Managing Director, UBA Cameroon, Mr. Georges Wega; Managing Director, UBA Capital Europe, Mr. Ebele Ogbue, and Head UBA Representative Office Paris, DeMontalivet Edouard.

Explaining the rationale for the awards, The Banker said UBA was chosen on account of the bank's successful turnaround to profitability following the write-offs and clean-up of its balance sheet in 2011.

The Banker also stated that other reasons for the selection of UBA were the notable consolidation of UBA's business across Africa as well as its innovative cross border payment/remittance products and platforms, which have made trade and financial transactions easier and more convenient in the continent.

Commenting on the awards, Group Managing Director/Chief Executive Officer, UBA, Mr. Phillips Oduoza, said it validated the UBA Group's "leadership position in the continent and lent credence to its expansion across Africa and promotion of industry wide excellence in the global banking community."

Oduoza added: "Winning the three highly coveted awards is a great accomplishment and it also reinforces UBA's collective steadfastness and drive to becoming the bank of choice in Africa, in line with the Bank's positioning as Africa's global bank. We dedicate it to all our stakeholders across the continent and are further encouraged to work harder."

Mmmmm ! People are left wondering if it is not hate mongering conspiracy
with racial discrimination toppings which motivates MCcain with team in the
the GOP Republicans to fight Susan with such lethal venom. Things cannot
continue to be this way........It is their way or no way.......Surely, wont they
get some reality check, so they engage on repairing their damaged image
after such a humiliating defeat.....??? They lost because they did not have
any reasonable substantive issues to sell to the people. It is time they be
realistic and get some reality check so they join the rest of people in Nation
building. I have watched Susan Rice and seen a highly qualified woman with
conviction doing her work at the UN Office with dedication, passion and with
Grace. She is one of our own we are very proud of. The attack on Susan is
quite unfortunate because she used the talking point she was given by the
CIA. This confirms that she is not the one the GOP should go after. To me,
Susan Rice has outshined many men and this could be another reason she
is under attack.........
 
 
 
Divided we fall, United we are strong and together we shall transform the
world to be a better place free from pain and sufferings.......
 
 
 
We need each other people, and we learn from our mistakes and No One
is perfect......Give Susan Rice a breather and let us all move on aiming at
sharing to improve matters......

McCaine obviously didn't think Condoleeza Rice was responsible for her misinformation about weapons of mass destruction in Iraq since he had no problem voting for her confirmation. This is just personal political attack against the Pres and they are using his Susan Rice because they do see her as an easy target. McCaine and racist Graham are just disgusting and despicable people to be using the death of Americans to try to bring the Pres down.Like Romney they will fail too

President Barack Obama praised embattled U.S. Ambassador to the U.N. Susan Rice as "extraordinary" on Thursday as she looked on during a Cabinet meeting at the White House.

"Susan Rice is extraordinary. Couldn't be prouder of the job that she's done," Obama said in response to a question from a reporter. The president and the assembled top aides then applauded.

But Obama gave no clue as to whether he was any closer to nominating her—or anyone else—to succeed Secretary of State Hillary Clinton. Clinton has said she will be leaving her post after four years as America's top diplomat, a role in which she has won praise from Democrats and Republicans in Congress.

Rice, who is known to be close to Obama, is thought to be a top contender for the job. So is Democratic Sen. John Kerry, who chairs the Senate Foreign Relations Committee. But she has become a lightning rod for Republican criticism of the administration's handling of the Sept. 11 attack on the American compound in Benghazi, Libya. Several top Republicans, including Sen. John McCain, have vowed to block her confirmation.

That assault claimed the lives of Ambassador Chris Stevens and three other Americans. Republicans have accused the administration of misleading the public by playing down intelligence that it was a terrorist attack. They have also charged that the administration ignored requests for increased security in Benghazi and expressions of concern about extremists operating in the eastern Libyan city.

The White House flatly denies deliberately misleading the public.

                WASHINGTON (AP) — With congressional opposition softening, U.N. Ambassador Susan Rice could find her name in contention as early as this week to succeed Hillary Rodham Clinton as secretary of state. It's a step that may signal greater U.S. willingness to intervene in world crises during President Barack Obama's second term.

                As Obama nears a decision on who should be the country's next top diplomat, Rice has emerged as the clear front-runner on a short list of candidates that many believe has been narrowed to just her and Sen. John Kerry, despite lingering questions over her comments about the deadly Sept. 11 attack on a U.S. Consulate in Libya.

                According to congressional aides and administration officials, Rice will be making the rounds on Capitol Hill this week for closed door meetings with key lawmakers whose support she will need to be confirmed. Those appearances follow her first in-depth explanation of her Benghazi remarks that Republicans seized on as evidence of the administration's mishandling of the attack that took the lives of the U.S. ambassador to Libya and three other Americans.

                Acting CIA Director Michael Morell will join Rice in her meetings with lawmakers.

                A senior Senate aide said the administration was trying to measure the strength of the Republican opposition to a Rice nomination, sounding out the more moderate members of the Foreign Relations Committee such as Sen. Bob Corker, R-Tenn., who is in line to become the panel's top Republican next year, and Sen. Johnny Isakson, R-Ga.

                Rice is scheduled to meet on Tuesday with Sen. John McCain, R-Ariz., her most vocal critic on Capitol Hill. She will also meet with Sen. Kelly Ayotte, R-N.H. McCain and Ayotte are members of the Armed Services Committee.

                During an interview on Monday, McCain said he would ask Rice "the same questions I've been talking about on every talk show in America." Asked whether he thinks she's still unfit for secretary of state and what he was hoping for, McCain interrupted and said, "I'm not hoping for anything. She asked to see me and I agreed to see her."

                Assessing the prospects for Rice before Obama makes any announcement would avoid the embarrassment of a protracted fight with the Senate early in the president's second term.

                On talk shows the weekend following the attacks, Rice relied on talking points provided by the intelligence community that described the attack as a spontaneous assault growing out of a protest of an anti-Muslim film. GOP critics say her remarks downplayed evidence of an obvious terrorist attack just weeks before the Nov. 6 election.

                Republicans called her nomination doomed, leading to a vigorous defense of her by Obama in his first post-election news conference. But since then, GOP lawmakers seemed to have softened their views. McCain, who said earlier this month that would he do everything in his power to scuttle a Rice nomination, said on Sunday that he was willing to hear her out before making a decision. McCain ally Lindsey Graham, R-S.C., has also eased his opposition and said he is usually deferential to presidential Cabinet picks.

                Sen. Jim Inhofe, R-Okla., a member of the Foreign Relations Committee, had issued a statement highly critical of Rice on the day of Obama's news conference. He indicated Monday that perhaps she didn't know what had transpired in Benghazi on the day of the attack.

                "I assumed she had full knowledge of everything that went on. I'm not at all convinced of that now. She very well could have been thrown under the bus," Inhofe said in an interview. He said she hadn't requested a meeting but he would be glad to meet with her.

                White House spokesman Jay Carney said Monday that the administration appreciated McCain's latest comments about Rice, but wouldn't say whether the president saw them as an opening to make the nomination. "Ambassador Rice has done an excellent job at the United Nations and is highly qualified for any number of positions," Carney said.

                Several diplomats currently serving with Rice said that what she lacked in Clinton's star power, she could make up with a blunter approach that demands attention and has marked her tenure thus far at the United Nations.

                Rice, who at 48 is relatively young, has played the role of "conscience of the administration" on human rights and detainee issues and would bring "a certain edge" to the secretary of state job, according one colleague who has dealt with Rice on multiple issues over the past three years.

                She "will not be going into the job as a star," said Karl Inderfurth, a former U.S. ambassador and senior State Department official who worked closely with Rice in President Bill Clinton's administration when she worked as a staff aide to the National Security Council and then as assistant secretary of state for African affairs. "She will be a rising star, though."

                "Hillary Clinton understood the politics of diplomacy: what the person across the table needs in order to sell something," said Inderfurth, now at the Center for Strategic and International Studies think tank in Washington. "Susan Rice's background is different. What she'll bring is her experience in multilateral engagement and the limitations thereof."

                "But the most important thing she brings to the table is her relationship with the president," Inderfurth said.

                Rice, like many other foreign policy experts of her generation, was shaped by the Clinton administration's inability to prevent the genocide of hundreds of thousands of ethnic Tutsis and moderate Hutus in Rwanda 1994. Years later, she told a journalist: "I swore to myself that if I ever faced such a crisis again, I would come down on the side of dramatic action, going down in flames if that was required."

                That doesn't mean the U.S. will change its policy of only providing humanitarian support to Syrian rebels fighting to overthrow the regime anytime soon. But Rice's confirmation as the next secretary of state could alter the balance in an administration that has viewed humanitarian interventions with significant skepticism, given its rejection of the Bush administration's war in Iraq.

                An early supporter of Obama's 2008 presidential campaign, Rice fell out with some of her Clinton administration-era NSC and State Department colleagues who urged her to support Hillary Clinton's competing candidacy, including her own mentor, Madeleine Albright, and some of her top aides.

                With the Clinton-Obama primary battle in full gear in April 2008, tension between the two camps was on public display at a ceremony and reception to unveil Albright's official portrait in the State Department's ornate 8th floor Benjamin Franklin room, according to several people present. At those events, Rice firmly brushed aside appeals that she switch allegiance, those present said.

                Obama's 2008 election brought with it the prospect that Rice, one of his campaign's top foreign policy advisers, might be in line for the Cabinet job she is known to covet. Instead, however, Obama went with the surprise choice of Clinton and gave Rice the U.N. portfolio, although he attempted to deflect her disappointment by restoring the job to a Cabinet-level position.

                But her sights remained set on the top job, according to people who know her.

                Since arriving in New York, Rice can point to a series of diplomatic achievements — most notably the NATO-led air campaign that toppled Libyan leader Moammar Gadhafi and tougher sanctions against Iran and North Korea over their nuclear programs.

                But Rice has also been criticized — along with other Security Council leaders — for the failure of the U.N.'s most powerful body to take action to end the 19-month civil war in Syria.

                She has also been criticized, especially by human rights groups, for being too protective of U.S. allies, namely Sri Lanka where the U.N. says up to 40,000 ethnic Tamil civilians may have been killed in the final months of the country's civil war that ended in May 2009, and Rwanda, which has been accused of backing the M23 rebel group that last week took control of the eastern Congo city of Goma.

                As U.N. ambassador, she has gained a reputation for a sharp intellect and sharp elbows. She is not known for diplomatic finesse, rather for being aggressive — sometimes too aggressive — and using salty language on occasion. In private, she has a good sense of humor.

                In a legendary exchange last December, Rice dismissed an appeal by Russia's U.N. Ambassador Vitaly Churkin, who also isn't afraid to speak out, for a Security Council investigation of civilian deaths in Libya from NATO's bombing campaign as "a cheap stunt" to distract attention from the Syrian conflict.

                "Oh, the bombast and bogus claims," she told reporters.

                Churkin responded by mocking Rice's Stanford University degree, saying: "We hear that the Obama administration wants to establish a dialogue with the international community in the United Nations... If this is the intention, really this Stanford dictionary of expletives must be replaced by something more Victorian."

                Some have suggested that his demands for a special Watergate-like select committee to investigate Benghazi (there are several other investigations already ongoing), stem from the fact that, in January, the Arizona senator will lose his seat as the highest-ranking Republican on the Senate Armed Services Committee, due to term limits.

                Thus, unless the Senate creates a brand-new select committee with extensive authority to investigate Benghazi — a committee where McCain would probably be the ranking member — the Senator could very well fade into oblivion, perhaps as the ranking member of the Indian Affairs Committee.

                Some have suggested an even deeper, more "sinister" motive — albeit a little less narcissist on the part of McCain, because it is "for the Party."

                By demonizing Rice and effectively sandbagging her Senate nomination for Secretary of State — "Borking" her, oh sweet revenge! — Obama's hand will be forced and he may nominate Sen. John Kerry for that position. Should this happen, voilà, Kerry's Democratic Senate seat is suddenly vacant and vulnerable.

                In a special election, Republican Scott Brown would have a fair chance of winning that important Senate seat. Rachel Maddow, for one, believes that this whole fight over Ambassador Rice is "all about the numbers" and an important factor in the Benghazi-Susan Rice "affair." According to the The New York Times, "Several senators, including Mr. McCain, said they would prefer Mr. Kerry and predicted that he would sail through a confirmation hearing."

                It is an interesting theory. However, we are straying from individual motivation, John McCain's, into the territory of group psychosis — the Republican Party's.

                Thus back to John McCain's motives for his Benghazi witch-hunt and his Susan Rice obsession.

                In all fairness to the Senator, we should not overlook that his motives and intentions may be as pure as the driven snow in Arizona.

                His concerns for truth, transparency and trustworthiness on the part of any administration, whether Democratic or Republican, and for the integrity of their staffs and of their nominees for critical positions in our government are unimpeachable, as evidenced by the following actions:

                When he vigorously opposed the case for going to war against Iraq, because it was based on faulty intelligence and concocted under false pretenses. A war that killed, not four, but more than four thousand of our troops.

                When he aggressively challenged and demanded investigations of the fabricated intelligence information about Iraq's military (WMD) capabilities that was provided to Colin Powell and which the general presented to the UN Security Council in February 2003.

                When he uprightly blocked the confirmation of National Security Adviser, Condoleezza Rice — who had made a false and public case for the Iraq war based on misleading information — as Secretary of State in 2005.

                When, once the scope of the shenanigans leading up to and occurring during the disaster that came to be the Iraq War became crystal clear, McCain consistently and tenaciously pushed for special select committees to investigate the reasons for such gross malfeasance and deception at the highest levels of our government.

                We may never know what motivates this "Maverick," but we can make some assumptions that will be called reasonable or outrageous, depending on the views — and sometimes the (political) motives — of those who make such judgments.

                Friends,

                Our roads have become the highest killer of Kenyans in recent times.

                Then we have those who get maimed for life, people who were very able and doing their work, then because of an accident, they get impaired and cannot fend for themselves anymore.

                Still, we have those who get hospitalized and when they come out, they have spent a fortune on their medication.

                This is why I fully support the New Traffic Rules and if it means walking to work, I will happily do that. Matatu Touts are not the only people on the road. All road users MUST adhere to the New Traffic Rules and for this, the Government must not relent.

                The Traffic Commandant Mr. Benson Kibue will be on NTV tonight. Let us listen to him and encourage him to be firm. He has our support as a people.

                Let us stand up in support of the Government on this one, and let us say NO to the Matatu tyranny once and for all.

                Switzerland tops the overall rankings in The Global Competitiveness Report 2011-2012. Singapore overtakes Sweden for second position. Northern and Western European countries dominate the top 10 with Sweden (3rd), Finland (4th), Germany (6th), the Netherlands (7th), Denmark (8th) and the United Kingdom (10th). Japan remains the second-ranked Asian economy at 9th place, despite falling three places since last year. 

                The United States continues its decline for the third year in a row, falling one more place to fifth position. In addition to the macroeconomic vulnerabilities that continue to build, some aspects of the United States' institutional environment continue to raise concern among business leaders, particularly related to low public trust in politicians and concerns about government inefficiency. On a more positive note, banks and financial institutions are rebounding for the first time since the financial crisis and are assessed as somewhat sounder and more efficient.

                Germany maintains a strong position within the Eurozone, although it goes down one position to sixth place, while the Netherlands (7th) improves by one position in the rankings, France drops three places to 18th, and Greece continues its downward trend to 90th. Competitiveness-enhancing reforms will play a key role in revitalizing growth in the region and tackling its key challenges, fiscal consolidation and persistent unemployment.

                The results show that while competitiveness in advanced economies has stagnated over the past seven years, in many emerging markets it has improved, placing their growth on a more stable footing and mirroring the shift in economic activity from advanced to emerging economies. 

                The People's Republic of China (26th) continues to lead the way among large developing economies, improving by one more place and solidifying its position among the top 30. Among the four other BRICS economies, South Africa (50th) and Brazil (53rd) move upwards while India (56th) and Russia (66th) experience small declines. Several Asian economies perform strongly, with Japan (9th) and Hong Kong SAR (11th) also in the top 20.
